School Year (one student per application).
Proof of residence is required for open enrollment students. Please see list below
- You will need ONE of the following:
-Signed Lease/Rental Agreement
-Mortgage Statement
-Property Tax Bill
-Deed - You will need TWO of the following:
-Payroll check stub (dated within past 30 days)
-Utility Bill (dated within past 60 days)
-Brown County Job and Family Service letter (dated within past 60 days)
-Recent Income tax return (dated within past year)
Deadline for accepting open enrollment students is May 26th. Applications received
after the deadline will be reviewed on a case-by-case basis by the Superintendent.

Completed form will automatically be emailed to Raegan White, Superintendent.
Approval will be sent via mail. Once approval is received, complete registration form,
gather necessary documents and make an appointment with the registrar to enroll.
Please note: Transportation is not provided for Open Enrollment Students unless
there is room on the route and students are able to board a bus at an established
bus stop, with the permission of the Transportation Department.
